Les tests de provocation bronchique dans l’asthme correspondent-ils à la réalité ? > Do bronchial provocation tests correspond to reality in asthma patients?

Résumé Depuis les premiers tests de provocation réalistes réalisés par l’équipe de Pepys en 1975 pour poser des diagnostics d’asthme professionnel, les tests de provocation bronchique (TPB) allergénique n’ont cessé d’être standardisés. Il est cependant important que ces TPB restent réalistes. Nous détaillerons un certain nombre de points importants à prendre en compte pour la réalisation de TPB réalistes : la qualité des extraits allergéniques jusqu’à la mise au point d’allergènes recombinants, le mode d’exposition aux allergènes, la taille des particules portant les allergènes, l’exposition concomitante à d’autres polluants auquel le patient est exposé en conditions naturelles. © 2003 Publié par Éditions scientifiques et médicales Elsevier SAS. Abstract Since the first bronchial provocation tests (BPT) done by Pepys in 1975 for the diagnosis of occupational asthma, BPTs with allergen have continued to undergo standardization. It is nevertheless important that these tests remain realistic. We will examine a certain number of important points that must be taken into account in order to obtain a realistic BPT: the quality of the allergen extracts and also any recombinant allergens used; the method used in the allergen challenge; the size of the allergen-bearing particles; and the concomitant exposure to other environmental pollutants to which the patient is exposed naturally. © 2003 Publié par Éditions scientifiques et médicales Elsevier SAS.
