Award/Upgrade Ticket Request• Pick the destination you want to visit and check the number of Qmiles you will need for each passenger using your most recent statement and the charts on our website, www.qmiles.com • Complete and fax this form to (+974) 4462 6313, or email it to membersvc@qmiles.com at least 7 working days in advance of your desired travel date to check on availability and make reservations. • The Member Service Centre will advise you when the tickets will be available for collection at your nearest Qatar Airways Ticketing Office. • Charges apply to cancellation, change of dates or change of routing once your tickets is authorized. Please check the Terms & Conditions on www.qmiles.com • Taxes and other fees apply. • Main member should provide original ID while requesting the award ticket / upgrade. • If someone requests the award(s) on behalf of the main member, the original ID of the main member and person requesting, should be provided. • All information is mandatory. • Award requests that are incomplete or illegible cannot be processed.
